The request for a job order request form can be made by email, fax or mail. Once the FLC Unit has received the form, it will be processed and a letter will be sent out with all pertinent information with a copy of the original form. The job order is mainly used for H1-B and PERM Programs.

All items need to be completed. Type or Print Clearly on the form. Dates of ads need to be stated in the upper right hand corner of the form. Instruction in retrieving the job order will be stated on the job order letter.
