The Office of Workers' Compensation administers and enforces state laws, rules and regulations regarding industrial accidents and illnesses.In accordance with 19 Del. C. § 2322E and effective 5/23/08, health care providers shall complete and submit an employee condition and limitations report
"Provider Form" to the employee, employer and employer's insurance carrier (if applicable) not later than ten (10) days after the first evaluation/treatment and then subsequently to report changes. Within 14 days of the injury notice, employers shall submit
a report "Employer Form" that provides health care providers and employer carriers (if applicable), an outline of modified-duty jobs available to the employee. Health Care Provider Form

The Health Care Advisory Panel in accordance with 19 Del. C. § 2322D, has proposed a certification process for the Health Care Provider Application
for Certification. This proposal sets forth the Medical Certification process and form for providers treating workers' compensation cases. Health Care Provider Application for Certification Form

On November 14, 2007 the Health Care Advisory Panel submitted to the Secretary of Labor for consideration the Delaware Workers' Compensation Health Care Payment System Payment Rates for Physicians and Hospitals "Fee Schedule." This Fee Schedule is part of the Health Care Payment System with an effective date of May 23, 2008.
